Come and learn lifelong habits to help you eat healthy and be physically active!

Take Control is an 8 session chronic disease prevention program that provides strategies to help adults manage their health. It informs, empowers, and motivates participants to change their eating and physical activity patterns. The program provides opportunities for participants to track their goals and progress toward healthy eating and physical activity behaviors. Each 1.5-hour session contains a basic cooking demonstration of a healthy recipe. The recipes and nutrition handouts are provided to program participants.

Program Benefits

• Recipes, water bottle, spice jar, and much more for you to keep

• Information to help you control sodium, fat, and added sugar

• 8 engaging and interactive sessions

Sessions will be held on Wednesdays from 10:30 AM to 12 Noon at the Agricultural Building, Auditorium B 1450 Fairchild Rd. Winston -Salem, NC 27105.

Registration is required and you MUST meet one of the following criteria to be eligible to participate:

  • Be a SNAP (Food Stamp) recipient OR
  • Be SNAP eligible OR
  • You gross incomes is at or below 185% of the Federal Poverty Line OR
  • If you receive services from the following entities: public housing developments, congregate nutrition sites, county health department programs like Wise Woman, WIC, Job Link/Work First participants, shelters (homeless or domestic violence), Re-entry programs (formerly incarcerated persons living independently), food pantries, and soup kitchens.
